For me, sometimes just an itchy red spot just fine. Sometimes it is natural TINY 1.4 bladders filled with liquid, a group no larger than the eraser end of a pencil, which broke out in 6 hours to leave a small ulcer that scabs heal in 3 days.

This is a picture of shingles or herpes zoster, which was directed, as it can cause blistering. I have never seen anything worse than that - and I'm not crazy.
